...that if your Spouse is employed, your family may also have Health and Dental Benefits with your Spouse's Employer? If so, it is your responsibility to make sure Claims are submitted properly to our Plan and to your Spouse's. This will ensure that your Claims are not delayed. It may also save our Plan unnecessary expenses, making money available for other Benefits! It works like this:
To avoid delay of your claims remember to show on your Claim Form whether your Spouse has a similar Benefit. |

Effective October 1, 2003 Active Plan Members have a $50,000 Life Insurance Benefit covering death due to any cause? In addition, if the Member's death is due to an accident, the Member's Beneficiary will receive an additional $25,000 - for a total amount paid on Accidental Death of $75,000. Effective October 1, 2003 Retired Plan Members who contribute at least $0.88 per month to the Welfare Plan and are receiving a Monthly Pension from the Pension Plan have a $10,000 Life Insurance Benefit. Check with the Administrator to confirm your coverage. |

...that your Member Information Card must be filed with the Administrator? If this Card is not filed and kept up to date:
By not filing a Member Information Card, your Claims will be delayed. Also, if you die your Death Benefits may be paid to your Estate. (Unnecessary Legal Expenses are paid by the families of Members who do not file Member Information Cards with the Administrator.) Be sure your Member Information Card is in the Administrator's Office and that it is updated every time you have a change in your Dependants - which may be through the birth of a child, change in marital status, or a death. By keeping your Member Information Card up to date, you are ensuring that your affairs are in order and that your Claims can be paid promptly. |
